Mafia es un juego de acción en tercera persona. El jugador asume el papel de un gángster en una ciudad norteamericana de la década de 1930, donde tiene la oportunidad de vivir a través de un gángster. El jugador va a experimentar todo lo que estaba ocurriendo en el mundo subterráneo durante este período tormentoso: alta velocidad, persecuciones de coches, la piratería, los asesinatos y atracos a bancos.
